The result of optimizing communication between different groups of professional was to improve medication appropriateness and patient-relevant health outcomes for residents of nursing homesCITATION Mah \l 7177 (Mahlknecht, et al., 2017). The study involved 300 nursing home residents aged 65 years and above and members of the different professional groups consisting of general practitioners, nurses and pharmacists in nursing home care.
