Pt reports constant pain of 9/10 in left scrotum reported as feeling like being punched. Nothing makes the left scrotum pain better or worse and no medications for pain have been attempted. Left groin pain reports as superficial/deep pain 9/10 that is getting worse. No direct impact or trauma to left groin reported. Associated symptoms include nausea with one episode of vomiting since the pain began. Nothing makes the N and V better or worse. Formal operational stage: patients uses symbols to relate to abstract concepts and able to make hypothesis and grasp abstract concepts and relationships. Current Medications: NoneAllergies: NKDAPMH: No History of major childhood illnesses. Has had regular checkups with pediatricianTympanostomy tubes as a young childNo hospitalizations, surgeries, ER visits, or specialist provider visits since last well visit.
