The client states he was stable for many years until recent multiple psychiatric hospitalizations. He was supposed to follow up with his psychiatrist in March but ended up back in the hospital. The client says he was admitted to Morristown MedicalCenter's Behavioral Health unit. The client resigned from his job five months ago due to a lack ofmotivation, and he expressed difficulty maintaining jobs lately. The client is in school for medical coding and billing. The client resides with his parents, is single, and has no children.Past Psychiatric History : G eneral Statement: Patient has history of Paranoid Schizophrenia, Major depression disorder diagnosis and Anxiety at the age of 19.
