It is critical to maintain adequate communication between physicians, nurses, technicians, pharmacy and patients at all times. It is very important for the safety and wellbeing of a patient that reports are thorough for proper care and transfer of care. An adverse event that I am familiar with while working in healthcare is improper documentation in a patient’s medical chart, when they are not regularly updated, and can happen due to the ratio of nurses to patient. “Current research indicates that ineffective communication among health care professionals is one of the leading causes of medical errors and patient harm.
