DOB: 6/10/2005 Gender: Male Marital Status: Single Race/Ethnicity: Caucasian Informant: Patient, R.R. and the patients mother Reliability: The patient is a reliable historian Chief Complaint: c/o Acute Joint Pain History of Present Illness: R.R. is a 15-year-old, single, Caucasian male who presents with acute pain that he rated 6 on a scale of 0-10, and swelling to his left knee. The onset was yesterday at the beginning of soccer practice, and he reports that he heard a loud pop just prior to the onset of initially sharp pain to his left knee. He states he is currently experiencing intermittent pain that is aching in character, with the pain being worse at night. He also reports stiffness of his left knee when he wakes up in the morning, and difficulty straightening his knee. He states that aggravating factors are bearing weight on his left leg
