HPI P.P. is a 25-year-old Caucasian female presenting for an office visit for initial evaluation for her mental health. She is not on any current medications. The patient feels that she doesnt need medications. She admits to a history of taking medications and then stopping them due to the way they make her feel. The patients states that she has depressive episodes at least 4-5 times a year. During this time, she doesnt want to get out of bed, has no energy, lack of motivation, feelings of hopelessness and no creativity. Her appetite increases dramatically and sleeps more than 12-16 hours a night. Her friends described her behavior as being depressed. This behavior starts after 4-5 days of no sleeping and working continuously on her activities with writing, painting and music. The patient also discusses her manic episodes.
